Summer 2021 Learning Session: Comprehensive Medication Management with Health Plans

The California Right Meds Collaborative, in partnership with Los Angeles County Department of Public Health, L.A. Care Health Plan, Inland Empire Health Plan, and Brand New Day Health Plan, hosted the Summer 2021 Learning Session on Sunday, June 27, 2021.  This is the fourth in a series of CRMC Learning Sessions, designed to prepare or advance the delivery of Comprehensive Medication Management (CMM) services, primarily through community-based pharmacies but for any outpatient practice. Topics varied from physician leadership and perspectives, generating disruptive ideas, the CRMC journey, patient engagement and activation, advanced clinical skills for diabetes and hypertension, video telehealth for best practices, psychiatric pharmacy essentials for primary care and more. Home BP Telemonitoring and Pharmacist on the Care Team to Improve Hypertension Control

This online training module created through a partnership between the USC Gehr Center for Health Systems Science and Innovation and The Right Care Initiative, will introduce the importance of utilizing team-based care approaches, specifically adding a pharmacist to the care team, to manage uncontrolled hypertension. Participants will hear from Drs. Margolis and Rehrauer from HealthPartners Institute on the work they’ve been doing with project HyperLink where they have utilized both telemonitoring and including a pharmacist on the care team.
